17 year old 0 gravida - virgo referred by gynecologist for a large cyst of the right ovary. She has no health problems, sometimes pelvic pain. Her menstrual period is quite regullar but fairly strong. Transrectal examination shows elastic round cystic formation reaching cranially up to umbilicus. According to the ultrasound examination - anteverted uterus of normal size, left ovary not detected, anechogenic cyst 12 cm in diameter from the right ovary. Cyst contains also an echogenic part towards it's caudal pole. Tumor markers (Ca 125) are negative. Diagnostic laparoscopy was performed. Right ovary measured 12 - 13 cm in diameter, left ovary was enlarged as well with a cyst 2 cm in diameter, uterus of normal size and both uterine tubes with no abnormalities. Lavage of the pelvic cavity was done first, then the wall of the large cyst was incized and 1000 ml of clear liquid was drained. Afterwards the right ovarian cyst was exstirpated. The cyst of the left ovary was exstirpated as well. The exstirpate tissue was send for histology confirmation but the presence of hair and adipous tissue suggested bilateral dermoidal cyst. Histology confirmed mature cystic teratoma of both ovaries with a cartilige in the cyst wall of the left ovarian cyst. Cytology came out negative.
