33 year old primigravida primipara arrives for 5 weeks amenorrhoea, positive pregnancy test, light bleeding and pelvic pain which started yesterday. Her menstrual cycle is otherwise regular. She sufferes from bronchial asthma.Bimanual vaginal examination shows light vaginal bleeding, retroverted uterus which is not tender to palpation, left adnexal region not painful but right adnexal region tender to palpation. Transvaginal ultrasound: retroverted uterus of normal size, endometrium 5 mm, no gestation sac in the uterine cavity. Both ovaries of normal size and shape, on the right next to the uterus an anisoechogenic formation 2 cm in diameter, small amount of free fluid in the pouch of Douglas. Serum - hCG level 4053 IU/l. Diagnostic laparoscopy was performed for the suspicion of the ectopic, right tubal pregnancy. Uterus was of normal size and shape, left ovary in adhesions between pelvic wall and colon, left uterine tube was in adhesions with left ovary. Right ovary in adhesions with the right uterine tube which was livid and dilated with an ectopic pregnancy. Right salpingectomy was performed. Left uterine tube was released from adhesions. Histology confirmed right tubal pregnancy.
