45 year old II gravida II para referred by gynecologist for a persisting cyst of the left ovary. She underwent laparoscopic exstirpation of the left ovarian cyst 6 years ago. 2 spontaneus deliveries 17 and 15 years ago, has regular menstrual cycle and suffers from hypertension. Has no strong pelvic pain but feels some pelvic pressure more on the left. Bimanual pelvic examination describes elastic formation in the left adnexal region 4 to 5 cm in diameter, anteverted uterus of normal size and right adnexal region no abnormal findings. Tumor markers (Ca - 125)-negative. Transvaginal ultrasound: left ovary with the anechogenic cyst 40 mm in diameter, right ovary of normal size, anteverted uterus, endometrium 2 mm. Diagnostic laparoscopy was performed. Uterus of normal size, both ovaries of normal size with no abnormalities, next to the left ovary is unsuspicious cyst parallel to letf uterine tube which measures 40 mm in diameter and is partly included in the left uterine tube. It is possible that the paratubal cyst was formed after previous ovarian cyst exstirpation. Paratubal cyst was exstirpated together with the part of the uteirne tube and left salpingectomy was performed. Histology result confirmed paratubal serous cyst.
